For An Absence (Please call in the following information before 10:00 a.m.)
Please state clearly:
Student's full name (Please spell last name)
The date of the absence
The reason for the absence
Your relationship to the student
A phone number where you can be reached
For Late Arrivals
Please call by 10:00 a.m. or send a note with the following information:
Student name (spell the last name or print)
Time student will arrive
Reason why they are late
Your signature
For Early Departures:
When information is given to us to contact your student the information will be given at the begining of the following hour. If it is an emergency situation that your student be contacted we will go the classroom and contact your student personally.
A note or a phone call must be given to attendance before school starts with the following information:
Student's name
Reason for the absence
Time student should be released from class
Your signature
Prearranged Absences: (These are absences that you know of in advance)
Please come to the attendance office for a Prearranged Absnece form. This form needs to be completed and returned to the attendance office one week before your absence.
Prearrange absences can be for:
Vacations or extended weekends
College visits
Hunting trips
Surgery
