EXTRACURRICULAR GUIDEBOOK (Includes Extracurricular Code) -- All-inclusive guide to athletics at Cedarburg High School. Contains information about such topics as Athletic Department philosophy, detailed sports information, registration instructions, WIAA guidelines, parent/coach communication, sportsmanship, awards, contest transportation, etc.

ATHLETIC REGISTRATION FORM -- Includes the Parent/Athlete Guidebook Agreement, and the Release Waiver, and Indemnification of Liability, Emergency Treatment/Participation Clearance, and Student's Health Information. Must be completed and turned in with the Activity Fee ($55.00) for each sport participated in (family cap of $275.00). For information only. Please use original form available in both the Athletic Office and High School Office.
TRAVEL RELEASE FORM -- This form must be completed, signed by the parent/guardian and returned to the coach before a student is cleared to travel with his parent/guardian. Under no circumstances will permission be given allowing athletes to transport themselves to or from a contest. Serves for only the date and contest listed on the form.
ACTIVITIES REGISTRATION FORM -- For all non-athletic CHS Extracurricular Activities, including Debate, Forensics, Drama, Pep Band, Pit Band, Academic Decathlon, Computer Team, Math Team, and Mock Trial.
CEDARBURG TRAVEL GUIDE -- Prepared for use by students, parents, and fans to promote and facilitate travel to athletic events at North Shore Conference and non-conference schools, specific off-campus sport locations, and (in the printed booklet) Milwaukee area high schools and UW-Madison campus.

The schools are grouped and coded by conference, with the numbers on this list corresponding with the numbers on the area map. (Location of off-campus field is listed only if team plays football off-campus.)
