Bullying, Harassment, or Discrimination Reporting
Student, staff, and visitor safety is our number one priority at Cedarburg School District. If you believe you have been bullied, harassed, and/or discriminated against by an individual on school grounds, report the incident by using one of the appropriate forms below. Please describe as best you can exactly what happened to you. Include dates; times; locations; the names of everyone who was involved in the bullying, harassment, and/or discrimination, including all person(s) who saw or heard what happened; and, all other relevant facts. If there was more than one incident of bullying, harassment, and/or discrimination, please describe each incident separately.
The confidential reporting forms may be completed and submitted in one of two ways:
The pdf version of the student and parent/visitor forms below may be submitted to the building principal and the employee form may be submitted by email to either CSD Complaint Compliance Officer, Tony DeRosa or Kirstin Collins.
OR
The Reporting Google Forms below may be completed online. Submitting this form forwards the information directly to the appropriate administrator.
This communication allows the appropriate school administrator to address concerns by providing assistance to those in need and taking any other actions the situation may deem necessary.
