All incoming freshmen will need a WIAA physician signed physical form.  Each subsequent year a physical or alternate physical card (filled out by a guardian) is required to be uploaded during the athletics registration process.  Only one card will be accepted for upload - so please be sure to upload the correct card.  Parents can check which physical card their child needs each year by going into Skyward into the left hand bar which reads:  “Portfolios and Reports”.  Clicking on this will show a listing of uploaded items including previous years' physical cards. WIAA Preparticipation Physical Evaluation Clearance Form (Parent AND Doctor Signatures required) for a physical examination taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year.

This form MUST be completed, signed by the parent/guardian, and returned to the coach before a student is cleared to travel with his parent/guardian, and returned to the coach after the contest.
